North Kingstown Family Practice has joined with other Ideal Medical Practices to seek ways of improving medical care for all patients. To do this WE NEED YOUR HELP!  The HowsYourHealth on line survey is a wonderful way of assessing your health as an individual, and the more people who do the survey, the more Dr. Ho can adjust our practice to make it better. If you receive your medical care from us, please take the time to go through the survey at least once every several years by clicking here on the How's Your Health link and using Access Code: IMP107.  At the end of the survey, it will ask if you would like to send your action form directly to your physician:  please click the "Submit info" button so we can review your health information at your next visit.
